What is a varicocele and what are its symptoms?
A varicocele is a vascular condition manifested by excessive enlargement and dilation of the veins within the scrotum. It often resembles varicose veins. In most cases, it is visible on the left side as testicular swelling, although bilateral varicoceles are not uncommon. Dilated veins—varicoceles—can cause tenderness, and some patients experience uncomfortable pulling sensations or pain in the scrotum. A frequent symptom is reduced fertility or complete male infertility. This issue is caused by excessive overheating of the testicles by the dilated veins or by impaired nutrition of the testicular cells. A consequence can also be testicular hypotrophy, which causes gradual shrinking of the testicle and reduced testosterone production. Last but not least, a varicocele can be an aesthetically disturbing element that negatively affects a man’s sexual life. If it is bothersome or reduces fertility, the best solution is microsurgical surgery.
Diagnosis of Varicocele
There are several ways to diagnose a varicocele, all of which are painless. Initial identification consists of a physical examination of the scrotum while standing, during which the doctor checks for enlarged veins. To confirm the diagnosis, a Doppler ultrasound can be performed, or the doctor may conduct a semen analysis and determine testosterone levels.
About the procedure
Microsurgical surgery using the Marmar technique is an effective way to treat a varicocele, or varicose veins of the scrotum. Treatment is necessary if pain is present or if the appearance of the testicles changes and becomes aesthetically unpleasing. Treatment may also be required in cases of fertility issues. If a semen analysis shows low or no sperm count, or poor quality, a varicocele may be the cause.
Before Surgery
Before microsurgical varicocele surgery, a preoperative examination is required, which can be provided either by your general practitioner or performed by us. Prior to the procedure, it is necessary to shave the scrotum, the area around the penis, and the groin. After microsurgical varicocele surgery, we recommend 5–7 days of medical leave, depending on the nature of the patient’s work. The surgery is performed personally by Milan Gajdoš, MD, MPH, an experienced urologist with over 30 years of practice, or Michal Polák, MD, a certified pediatric urologist. At our facility, you are guaranteed an individual and friendly approach; your attending physician is available to you at all times. Furthermore, you have the option to communicate with them by phone 24 hours a day or in person until full recovery. At both of our locations, you have a guarantee of the highest quality materials and strict hygienic standards. Our modern facilities guarantee discretion, unlike overcrowded hospitals.
After Surgery
The surgery is performed under general anesthesia and usually lasts a maximum of 60 minutes. After the procedure, the patient goes home the same day. The procedure does not require hospitalization. Due to the general anesthesia, we recommend arranging for an escort. The procedure requires 5–7 days of medical leave (depending on the physical demands of the patient’s work). Wound care is very important. It is necessary to change the dressing regularly for the following 7 to 10 days. With each dressing change, the wound must be cleaned with Betadine solution, dried with sterile gauze, and treated with Framykoin or Bactroban ointment. Patients return to our clinic for a check-up in 10 to 14 days, as agreed with the doctor.
Benefits and Risks
Microsurgical varicocele surgery carries minimal risks. Only exceptionally can chronic or intermittent scrotal pain occur. Similarly, testicular atrophy is rare. The advantage of microsurgical surgery is its high effectiveness in eliminating pain and improving fertility. The recurrence rate of symptoms is less than 1.5%, while with conventional surgery it occurs in up to 30% of patients, and with the laparoscopic method, it is approximately 20%. Other advantages include a much smaller scar and a lower risk of secondary hydrocele, which is a common late complication after varicocele surgery.
